Questions for Child's Orphanage/Doctor/Caregiver

This is a list of questions I obtained from multiple resources. Please feel free to use any or all that would benefit your child. If you have questions you would like to see added, please click HERE.

Questions for Caregiver / Orphanage Doctor
Print these questions and then record the answers in your notebook. You will be able to ask them the first or second day. If you have a video camera, ask if you can record the voices without picture in case you miss anything.
  • About Child General
    • What is his normal sleeping routine? (bed time/ wake up / naps)
    • What does he eat?
    • What does he play with?
    • What can he say?
    • Does he take a pacifier?
    • Does he take a bottle or sippy cup?
    • Can he feed himself?
    • What size clothes does he wear?
    • What size shoes does he wear?
    • Is he potty-trained? If not, what size pull-up/diaper?
  • About Child History
    • Why was he placed in the orphanage?
    • Why were the parental rights terminated?
    • Where was child born (city/hospital)?
    • What time was child born?
    • Were there any complications during his birth?
    • Did his birth mom see him/hold him?
    • Who else was present during his birth?
    • Will his birth mother know anything about us?
    • Will his birth mother know anything about him?
    • Who named him?
    • What is his ethnic racial background?
    • Does he have any other siblings?
    • Will his siblings know about him/us?
    • Does anyone in his birth family know about him?
    • Who knows what?
    • Any other relatives they know about – names & locations?
    • Is he baptized?

    • What are her/his first, middle, and last names?
    • How old is she/he?
    • Where is she/he from?
    • Is/were she/he married?
    • Did they go to college?
    • What do they look like?
    • Is it possible to have pictures?
    • What religious background did they have?

    • Do they have any special hobbies or interests?
    • Occupation?
    • Any medical issues?

    • Was his father ever involved?
    • If child wants to meet his birth parents, can it be arranged?
  • Medical Questions
    • Is he allergic to anything?
    • What exactly is his diagnosis?
    • Is there any special care he needs?
    • Are there any foods he cannot have?
    • What immunizations has he received
    • Was he exposed to anything prenatal?
    • Was he exposed to anything postnatal?
    • Was he exposed to HIV or other STD?
    • Are there any physical or mental illness in this family history?
      • Heart disease
      • Cancer
      • Diabetes
      • Asthma
      • Allergies
      • Depression
      • Alcoholism
      • Drug Addiction

      • Vision or hearing impairment
    • Is he on medications?
    • Is he at a normal developmental level (physical and mental)?
